Capital Health Plan Medicare Advantage (HMO) plans provide all your Medicare benefits and services, including prescription drug coverage (Medicare Parts A, B, and D). It's easy to find the plan that is right for you.
To enroll in a Capital Health Plan Medicare Advantage plan, you must be entitled to Medicare Part A and enrolled in Medicare Part B. You must be a permanent resident of our service area, which includes the Florida counties of Calhoun, Franklin, Gadsden, Jefferson, Leon, Liberty, and Wakulla.
You are now able to submit feedback about your Medicare health plan or other non-urgent issues directly to Medicare using the Medicare Complaint Form. Additionally, the Office of the Medicare Ombudsman (OMO) helps you with complaints, grievances, and information requests about Medicare.
NOTICE: Medicare allows you to change health plans during the Annual Enrollment Period from October 15 through December 7. If you do not want to change health plans you do not need to do anything at this time. However, Capital Health Plan does offer other plan options at a lower cost to you if you are considering changing plans.
