The Physician Group of Capital Health Plan (PGCHP) is focused solely on taking care of Capital Health Plan (CHP) members. Whether you are a long-time patient or new to their practice, every aspect of your patient-centered medical care is coordinated by a primary care physician dedicated to improving your health.

The Physician Group of Capital Health Plan provides evidenced-based comprehensive health care for our patients of all ages, genders, and conditions. Our goal is to promote disease prevention and healthy living, so in addition to diagnosing and treating illnesses, we also provide preventive care, health risk assessments, immunizations, screening tests, comprehensive management of chronic conditions, and education on maintaining a healthy lifestyle.

In addition, the Physician Group of Capital Health Plan utilizes an extensive network of speciality care physicians located throughout our service area who provide care to our members in their own offices. CHP’s network also includes hospitals and other clinical facilities throughout the service area, and Centers of Excellence when medically appropriate.

One of the most important health care decisions a patient will ever make is selecting a Primary Care Physician (PCP). At the heart of a Patient Centered Medical Home is your ongoing relationship with a PCP and the Care Team. Your PCP at the Physician Group of Capital Health Plan will provide comprehensive health services through an integrated health care delivery system. Evidence-based medicine and clinical decision-support tools guide decision making. You are at the center of the medical team and actively participated in the decisions concerning your health.

Your immediate Care Team includes your PCP, nursing staff, and reception staff. This team is your health care partner and central coordination point for everything pertaining to your good health. Everything is coordinated across all elements of the complex health care system and communication is streamlined through the online secure patient portal.

Selecting a CHP Primary Care Physician

Choosing the right physician as your primary health care partner is an important decision. Evidence-based, patient-centered care is what the Physician Group of Capital Health Plan delivers. We offer a wide selection of board-certified, experienced, and caring physicians to address your health needs and goals. If you are a Capital Health Plan member and would like to become part of the Physician Group of Capital Health Plan, please follow the steps below to choose a primary care physician (PCP):

  1. Find a physician in the Physician Group of Capital Health Plan who is accepting new patients:
    • Access a list of our physicians on the Doctor/Provider Directory page.
    • Click on the link that best suits your needs, either by location or specialty.
    • Click on a physician's name or picture for a brief biography, board certifications, languages spoken, and whether they are accepting new patients.
    • Review the physicians accepting new patients and choose the one you would like to have as your health care partner.
  2. If you have trouble finding an available primary care physician, please call Capital Health Plan Member Services.
  3. After picking your physician, update your records either by calling Capital Health Plan Member Services or by making the selection in your CHPConnect account, which automatically sends a secure message to Member Services.

Our Services

Urgent Care

Urgent Care is a great alternative when your primary care provider is not available.

Medical conditions that might be treated at Urgent Care include: 

  • Upper respiratory infections such as colds, ear infections, bronchitis, and sore throats.
  • Minor but acute cases, including fever, nausea, vomiting, and urinary tract infections.
  • Minor trauma, including ankle sprains and cuts.

Call the Urgent Care office at (850) 383-3382 for an appointment before coming in. Calling is necessary to reduce the time you wait.

The Urgent Care office is not a walk-in clinic but will be accommodated next available. Also members may schedule their own same day appointment on the NextGen patient portal for selected minor illnesses or injuries. See Capital Health Plan Health Centers to enroll.

Nancy Van Vessem, M.D., Center for Healthy Aging

Nancy Van Vessem, M.D., Center for Healthy AgingThe Nancy Van Vessem, M.D., Center for Healthy Aging is designed to focus appropriate care to meet the specific needs of older adults and adults managing complex medical conditions, by focusing on the following areas:

  • Primary Care services utilizing Internal Medicine or Geriatric trained physicians whose practice will be predominately-older adults, providing a comprehensive and respectful approach to medical care. Members are supported by addressing the physical, social and emotional aspects of their medical care
  • The Fall Prevention Clinic will assess and stratify the fall risk of older adults with a one-on-one evaluation with a trained professional
  • Cognitive Screening Clinic will provide cognitive testing using clinically validated tools by certified testers, upon referral from primary care physician
  • Geriatric Assessment Program will provide a comprehensive evaluation by a care team, which includes a Geriatrician, a Geriatric Medical Social Worker and a Registered Nurse. The team will meet with referred members (along with their families/caregivers) and design care plans that allow members to work with their primary care physician to help reduce fall risk, medication burden, and manage memory loss or dementia, if present. (This program will be available in September 2022)

Colon Screening Program

Colorectal cancer is a cancer of the large intestine or rectum, and it is the second leading cause of cancer deaths in the United States. The primary goals of colon screening are to prevent cancer from occurin and to detect colon cancer early, when the change of survival is highest. The American Cancer Society recommends that everyone should start colon cancer screening beginning at age 45 and, according to the U.S. Preventive Services Task Force (USPSTF), screening should continue through age 75, and in some cases should continue through age 85.

Colon screening participation is easier than ever. With a stool testing kit, you can collect your sample at home and return it by mail, or deliver it to one of our Capital Health Plan Health Centers. For the test, only one small sample of stool is required and there are no dietary or medication restrictions. Afterwards, the test result is mailed directly to you. However, structural exam of the colon (i.e. flexible sigmoidoscopy, colonoscopy, or CT colonography) may be required for further testing. The benefit of having a flexible sigmoidoscopy or colonoscopy as compared to other screening methods is that growths can be painlessly removed during the exam, before cancer cells develop, therefore preventing colon cancer from occurring. Schedule your appointment today for your colonoscopy/flexible sigmoidoscopy by calling the Capital Health Plan Colon Screening Program.

CHP Infusion Services

Infusion Therapy deals with all aspects of fluid and medication administration, through a vein in the arm or directly into the skin. Special infusion pumps can be used for this purpose. Advantages of coming to CHP’s Infusion Center include ease of access for you and your provider, a comfortable setting, and a friendly staff.  

This procedure is performed to help facilitate the treatment and management of certain medical conditions.

Our Infusion services include, but are not limited to the following:

  • Access indwelling catheters for blood collection and IV administration
  • Antibiotics
  • Flushing and accessing ports 
  • Infusion therapy for various conditions
  • IV hydration
  • Routine flushing of central venous lines

Some examples of medical conditions that may benefit from Infusion Therapy at our center include:

  • Connective tissue disorders

Wound Care Center

Your skin is very important to you and to your overall health. Capital Health Plan is pleased to offer our members options for the treatment of skin and wound conditions. The Wound Care Center offers patients options for treatment of most skin conditions as well as most wound care services.   

  • Management and treatment of acute and chronic wounds
    • Arterial, venous, and diabetic ulcers/wounds
    • Surgical wounds
    • Pressure ulcers

    • Malignant cutaneous wounds

Musculoskeletal Medicine

At Capital Health Plan we realize that it is very difficult to be all that you can be and exercise as you know you should when suffering from acute or chronic pain syndromes. While in the past patients were typically referred to orthopedic surgeons for diagnosis and management of these conditions, many if not most of them are non-surgical problems or at least have potentially viable non-surgical solutions. Those options can be skipped or minimized when consulting with a surgeon. Therefore, in 2007 Capital Health Plan opened an alternative clinic at the Governor’s Square Health Center emphasizing non-surgical solutions when appropriate for many musculoskeletal conditions. 

We see patients by referral from the Physician Group of Capital Health Plan, as well as Capital Health Plan members whose primary care physicians serve in the Family Medicine Residency program at Tallahassee Memorial Healthcare. Initial consultations typically take less than two weeks from the time of referral and are affordable for the patient because we collect only a primary care copayment. 

Great care is taken to minimize any pain associated with injection by using topical ethyl chloride, very small bore needles and copious injectable anesthetics. We see patients with virtually any kind of musculoskeletal complaint except for those originating in the spine, from young to middle-aged athletes to older patients with debilitating arthritic conditions. We offer a full spectrum of injectable alternatives. However, we do not offer cervical, thoracic, or lumbar epidural injections or peripheral nerve blocks/ablation. If you are interested in receiving any of these services, please see your primary care physician for a referral.

To better serve you, we offer the following tests in our offices:

We also contract with LabCorp for all of our other laboratory testing needs. We have LabCorp testing centers conveniently located at our Centerville Health Center, Governor’s Square Health Center, and Metropolitan Health Center locations.

The Physician Group of Capital Health Plan believes it is important to have skilled and experienced triage nurses ready to help you with your health care needs.  Our patients may access the triage nurses by calling their primary care physician’s office phone number and following the prompts for a same-day office visit appointment. 

After hours, patients may also call directly to Urgent Care of Capital Health Plan (850-383-3382) to speak to a triage nurse.
