Commission statements and annual small employer group renewals will only be available on the CHPConnect Agent portal. As a new agent, please be sure you or an appropriate representative of your Agency, have completed the online CHPConnect registration and submitted the required Broker/Agent and Agency Agreement at the time you submit your completed Agent Appointment Application. Please contact the Sales and Account Management Team if you have any questions.
Please note that Capital Health Plan will only appoint an agent with a new group submission or an Agent of Record (AOR).
